Accessibility fixes and improvements

• MDL-88949 - Course index subsections expose duplicate treeitem roles
• MDL-88948 - Course index accessibility issues
• MDL-85690 - Keyboard navigation in TinyMCE's help modal is broken when the editor is displayed on a modal dialogue
• MDL-84065 - Keyboard trap when clicking "Skip Table of Contents"
• MDL-75699 - Moodle modal dialogues always use h5, which might not fit the heading structure
• MDL-73125 - Focus is lost after sortable_list dialogue
• MDL-72220 - Accessibility issues with date_time_selector form field
• MDL-89207 - Expanding and collapsing accordions in forms can result in wrong expanded state
• MDL-89384 - Allow visually hidden column headers in report builder

Security fixes

A number of security related issues were resolved. Details of these issues will be released after a period of approximately one week to allow system administrators to safely update to the latest version.
